No medication can be given to students by staff members without the proper paperwork.

Please print documents and bring them to the school or you may also fax the forms to our school at (919) 560-3949. Please do not send medication with your student. It is the parent's responsibility to turn in medication. 

In order for us to accept and administer any medication, the current year medication order form will need to be completely filled out, signed by a doctor and must match the description of the medication.

The form(s) must be turned in at the same time as the medication. 

At the end of the year, parents will have two weeks to pick up their student's medication. After two weeks, the medication administrators will dispose of the medication.
